The LYLA Study (Listening to You to Learn about re-Admissions) is an innovative UCSF Study to learn more about hospital readmissions in a timely manner, to close the feedback loop, and improve care.
No one wants to be readmitted to the hospital. To better understand readmissions when and if they occur, we need a full accounting of them. Unfortunately, if a patient is discharged from UCSF and readmitted at a different hospital, we (UCSF) don't necessarily know about it, and can't learn from it to improve care. The LYLA Study aims to partner with patients and families to make care better by giving study participants easy tools to use to let us know about readmissions if and when they occur.
Adult patients being discharged from UCSF will have the opportunity to participate. One option is to fill out a survey that will be sent 90 days after leaving the hospital to tell us if you were readmitted anywhere. This survey will be sent by email. The other option is to dwnload a UCSF built smartphone app called Eureka, that will automatically detect if a readmission has occurred. Users of the Eureka app have to do nothing other than enable the app.
